New Patient Psychiatry Appointment Request

Our lines can be busy but we will try to return your call within 48 hours. Should you not be able to reach us by phone, please leave a detailed voice message so we can get back to you.

• New patients please arrive 15 minutes prior to your appointment to set aside time to fill out the required new patient paperwork. You may also download the appropriate paperwork to fill out and bring with you to your scheduled appointment.

• New patients via telehealth, please go through the check-in process on the patient portal (Onpatient.com) and the required forms will be there to complete prior to your scheduled appointment time.

INSURANCE:

At the present time we are accepting new patients who are either self-paid patients or are members of one of the following insurance companies*:

Aetna PPO/EPO
Blue Cross / Blue Shield PPO

ComPysch

Lyra Health

PSYCHIATRY SELF PAY RATES

$400 for initial one hour evaluation

$200 for each additional follow up

***Please note that our providers do not perform psychological evaluations to diagnose ADHD/ADD and Autism. Currently our psychiatrist and PAs provide medication management only.
